The Pentagon's Inspector General has concluded an investigation into charges of sexual assault by cadets at the US Air Force Academy.
The report faults eight Air Force officers for failing to respond to complaints once they were filed.
The blame doesn't stop there. It says a series of commanders "failed to acknowledge the severity of the problem" repeatedly over the last decade.
The Air Force vice chief of staff says it won't happen again. General Michael Mosely says cultural change takes time, but he says the Air Force is committed to making sure that it does.
