The Occupational Safety and Health Administration has again cited a Birmingham company for safety violations, slapping the firm with more than quarter million dollars in fines.
U.S. Pipe and Foundry has been singled out by OSHA for repeatedly ignoring safety rules, even after being told to fix them after a fatal workplace accident in 2004.
Subsequent inspections found numerous safety problems including unguarded opening in floors, improperly stored gas cylinders and electrical hazards.
The company has 15 days to appeal the $236,000 fine.
